Your first visit,
with less uncertainty.
You do not need to have treatment figured out before you call. Start with your questions, learn what to expect and prepare for your visit. You can book directly without a dentist’s referral.
Complete your forms online.
Please complete your patient forms online before your appointment. Our team sends your personal Henry Schein One Forms link by text or email. Each link is unique to you, so there is no shared forms link on this website.
Look for the message from our office and use the link provided. If you cannot find it, or the link does not work, call us for help or a new invitation.
Call for help with your formsAlready completed your forms online? You do not need to fill out the paper versions too unless our team asks you to.
Unable to use online forms? Paper options
Online forms are our preferred option. Use these printable forms if you cannot complete the online forms or our team has asked you to bring paper copies. Print and complete the registration and medical history, review the privacy notice, and bring your paperwork to your appointment.

Room for your questions
“My consultation was very thorough and educating. I learned a lot and was able to ask all my questions.”
Before your first visit.
- Gather your questions and treatment goals.
- Have your current medication list and relevant health information available.
- Bring any referral information and ask about transferring recent dental records.
- Have insurance information available if you would like to discuss benefits.
Keep your appointment details, a short packing checklist and questions for your doctor together on one page. This optional planner is yours to keep; it is separate from the patient forms.

Your visit is a chance to discuss your concerns, review relevant history and understand the evaluation and next steps.
Do I need a referral to book?
No. You can contact our office directly to schedule an evaluation. Tell us who provides your regular dental care so we can discuss records and coordination.
I already have a dentist. Will I keep seeing them?
