Let’s connect with your office.
Leave your practice’s contact details and our team will contact you to coordinate a referral. For a time-sensitive case, call (661) 665-0077.
Office contact details only. Please do not enter patient names, dates of birth, treatment information or records here.
Your office contact details are sent to the front-office email through Cloudflare. This requests a callback; it does not submit a clinical referral or book an appointment. The team will explain how to send patient information securely.
Specialist care within your restorative plan.
Your patient’s periodontal health and planned restoration belong in the same conversation. Share the question you need answered so the evaluation can focus on what matters for the next stage of care.

Start with the case and the restorative goal.
Call (661) 665-0077 to discuss a referral or request a case conversation. Include your proposed restorative design, the clinical question and any timing constraints. For implant cases, Dr. Dreyer’s surgical planning is coordinated with the dentist providing the restoration.

Implant placement, surgical and nonsurgical periodontal treatment, soft-tissue and bone grafting, sinus augmentation and ridge grafting. Dr. Dreyer and Dr. Morcos provide periodontal and implant care.
When to consider a referral.
- Periodontal concerns: persistent inflammation, deeper pockets or bone loss that needs specialist evaluation, including reassessment after initial therapy.
- Recession and tissue concerns: exposed roots or soft-tissue concerns affecting comfort, cleaning or a restorative plan.
- Tooth replacement: implant planning, assessment of supporting bone or a question about grafting before restoration.
- Restorative access: evaluation for crown lengthening when more natural tooth structure may be needed for a restoration.
These are reasons to request an evaluation, not automatic indications for surgery. Include the clinical question you want answered.
Keep the next steps connected.
At referral, tell us which findings and updates your office needs and confirm how they should be delivered. The coordination plan can include:
- Consultation findings: the periodontal or implant evaluation, proposed options and implications for your restorative plan.
- Planning together: surgical and restorative roles, treatment sequence and when the offices need to confer.
- Treatment updates: care provided, healing considerations and findings that may change the next step.
- Return and maintenance: coordination with your office for restorative care and an individualized maintenance schedule, including alternating visits when appropriate.
Confirm the reporting format, secure destination and timing with our team. No fixed turnaround or maintenance interval applies to every case.
Your questions, answered.
Can I discuss a case before sending the patient?
Call the team to arrange a conversation. Sharing the restorative goal and the question you need answered helps focus that discussion.
How should we send radiographs?
Use the secure method provided by the practice. The fillable PDF helps organize a referral; it does not upload or transmit records.
Who provides the final restoration?
The dentist providing the crown, bridge or denture coordinates restorative care with the surgical plan. Identify that clinician and the proposed design when making the referral so the roles can be confirmed for the case.
